Wiki Does CPT 64721 bundle with CPT 25609?

There are no NCCI PTP edits between 64721 and 25609.

What denial code was used?
 
Which payer? Do they use McKesson edits? Sometimes those differe from NCCI and no modifier allowed (posssibly?)
What is the diagnosis on the CTR? If it's the fracture that could be why. The report has to have documented compression/neuropathy/CTS to report separately.
You may have to appeal with documentation.
