Wiki does the removal of old ICD generator get billed

buddy came in with single chamber ICD.
the RV lead was occluded and capped.
they put in new ICD can and new RV lead
does this get billed as 33249 for the new lead and can, and a 33241 for the removal of the old can?
OR is this something else completely?
