Is it correct the DOS must be on each page of the patient note? If a notation with vitals is entered on the page to include a date, is it correct this can't be assumed to also be the DOS?

It's my understanding that the date as well as the patient name must be included on the top of every page of the MR regardless of if the date is recorded elsewhere with in the document.
