Is it a requirement that the DOS must be on each page of the note no matter what?

example - electronic office note - each page has patient name, a page number; Page x - but as a stand alone cannot identify page x of what? and no DOS? Is it correct not to code this page(s)?

also has a date vitals were entered but still not DOS - am I correct not to code this page?
