Is it the surgeon's responsibility to tell us the level or is that something we should be figuring out. our head coder was more or less arguing with the manager about who's responsibility this is. our manager's final word is that we're going to have to figure it out because the surgeon isn't going to ... any thoughts?
 
Even if the provider assigns the level of service, it is still a coder's responsibility to verify that the level assigned is met by the documentation provided.
