I work for an orthopedic practice who has enlisted a company to come in and do a swab test on our patients who are taking pain medication. We are only doing these swab test on Medicare patients and billing medicare for this test. The patients have signed a waiver indicating that they know what we are doing. Medicare is reimbursing for these test, but the company is not billing the patients for the balance. Is this correct or should they bill the 20% to the patient or is it ok for them just to bill medicare only. I want to make sure that we are doing this correctly. Please let me know your thoughts.
 
To my knowledge, if they are contracted with Medicare they shouldn't be accepting "insurance only." The patient is obligated to pay their responsibility.
