Each vial is 50 mg. We only gave 138.18 mgs and we wasted 11.89 mgs. How do we report this as drug waste? Thanks.

1 unit is 1 MG so:
J1602 139 Units (round up due to fractional units not accepted)
J1602-JW 11 units (150 - 139 = 11)
You wouldn't want to round up the discarded otherwise you will be over billing by 1 unit.
The instructions that I see from CMS don't really address fractional units, other than cases where the HCPCS Unit size would cause over-billing
