Dual pacer w/removal of lead at same session

Hi...pt came in for a dual pacer, this was completed, but as pt was being closed up, the pt had a capture threshold that was all over the place, therefore, she removed the lead in the right ventricle and replaced it with an active fixation lead. Not sure how to code this. 33208...what else? Please Help!! thank you Sandy:)
