Wiki Dx 'always' requires path report???

One of my Doc's insists he can do a 'gross identification' of a lipoma and this does NOT require a path report to have it coded it as such. He states there is documentation with either The American College of Pathologists or The American College of Surgeons regarding certain benign lesions/tumors that do not require microscopic identificaiton for diagnosis. I have been all over both websites and can not find anything to prove or disprove. Please help.
