Hi!

Looking for some guidance if diagnosis code 764.00 can only be used for infants? Our physician has a 10 yr old that has a history of being born 'light for dates' w/o mention of fetal malnutrition. I don't see a history of code for this and our edit is suggesting this is only for pt's ages 0-12 months old. If anyone has advise on this either way I would appreciate the feedback.

If you read the information for Chapter 15 Newborn (perinatal) guidelines (760-779) in the Official ICD-9-CM Guidelines for Coding and Reporting, Section C Chapter Speciific Guidelines, it sounds like these are only to be used on the newborn record.
