Wiki dx code for a work insurance wellness visit

I have a patient who presented for a wellness exam and labs. These are required by her insurance company, through her employer. Do I code them V70.0 or V70.5? I see more and more of these types of visits and I am just not sure.
 
My opinion is V70.3. It specifically lists "General medical examination for: Insurance certification". Although the exam is through her employer, it is for insurance purposes.
 
I actually did code this one as V70.3 and of course, denied because of dx.. was looking at others and wondering if i should have used something else?
