What diagnosis should be used when a patient comes in to have immunization records reviewed and a form signed off by a provider? No exam is done.

The diagnosis should be the reason that the patient needed the physician to review the immunization history and sign off on a form (e.g., Z02.0, encounter for examination for admission to educational institution). Note that these encounters may not be covered by a health plan due to lack of medical necessity.
