I need help with a dx code for inadequate Essure placement. The doctor is doing a hysteroscopic removal of Essure device and then a bilateral Salpingectomy. The OR report states that on the hysterosalpingogram the correct configuration of the coils were not seen and sterilization could not be guaranteed.

My thinking is a 58562 for the removal of essure just not sure on dx and a 58661 z302 for the lap salpingectomy.

Any suggestions are appreciated.
