Wiki dx codes to support a 360 plif fusion

Hello, I have a neurosurgeon that performs a ton of these 360 plif spinal fusions with decompression of the nerve roots. I am trying to code the decompression with a modifier 59 to cover the foraminotomies required to free the nerve roots. Does anybody have any insight as to which dx codes are necessary to support this as it is normally bundled with an interbody fusion?

We've performed these for degenerative disc disease, spinal stenosis, spondylolisthesis, etc...

Thanks for the response. I feel good with the Stenosis and Spondylosis dx codes but I was concerned that the DDD might not be enough. I also have an OP report where the surgeon is only stating intractable lumbar pain due to a previous MVA. I am concerned that these claims could be rejected since the dx does not really address the compression element. BTW - I am new to this practice and so I am trying to gain as much knowledge in Neurosurgery as possible. Do you know of any good resource websites? Any information would be greatly appreciated.
