The patient is post mastectomy and had a palpable mass, but my doctor (a pathologist) says this is reactive fibrosis of the chest wall. Should I code this as V45.71?
 
As the pathologist clearly documents 'Fibrosis' of post mastectomy site, it can be coded as 709.2 and V45.71, but the depth of fibrosis is not documented clearly. Better to query the physician or refer the site from where the specimen has been taken to decide the depth. i.e, skin, bone etc...
