"Mild sequela of small vessel ischemic disease." Anyone have a clue how to code this? The only other enlightening information he includes in the findings is "Scattered periventricular and subcortical white matter T2 hyperintensities, nonspecific but likely related to chronic microangiopathy". Could I code it as small vessel ischemic disease?
