Wiki dx lap turns to laparatomy/myomectomy

not sure how to code, new to ob coding. dx lap turned to open laparatomy with bil salp/ooph and myomectomy/lysis of adhesions. any assistance would be appreciated

You can't bill for the dx lap it is inclusive to the other codes. I would bill 58720 for the salpingo-oopharectomy and 58140 for the myomectomy. This code is for 1-4 tumors if it is more you would use 58146. As for the adhesions this would be inclusive also. If your provider spent 20 minutes or more just on the adhesions and documented their time then you can add a 22 modifier to your primary CPT code and some insurance companies will pay you a little extra. You can use dx code V64.41 laparoscopy converted to open
