I need some help with locating the guidelines or how to code ICD-9 codes when "Vs" is used.
For example the doctor wrote:
BPH vs Prostate Ca
TIA vs CVA on evolution

Which dx should be coded and is there any guidelines or rules to it?
 
If you are coding for the physician then you do not code either dx you code only the symptoms, if you are coding for the inpatient facility then you code them both, read the ICD-9 coding guidelines for this information.
