Can anyone tell me if there are written guide lines for out patient coding when the physician states the DX as; DX vs DX or Symptom vs Symptom and nothing is confimed by the test.
I know that you are suppose to code symptoms but is it in writing any where and what do you do if the 2 are already symptoms and nothing can be confirmed. Thanks
 
If they are stated as symptoms then the patient must have complained of it or experienced it, so code the symptoms as they must exist or they are not symptoms.
 
an example would be Vaginal bleeding VS hematuria and neither have been confirmed. should I just code an unspecified hemorrage, or code them individually? Is there a guideline for this written any where? or would it fall under the guideline for R/O and inconclusive dx etc?
