Is a 25 modifier required on an E&M when an EKG (93005; tracing only) is performed?
 
Hi,

No need, if this EKG is more over significant to the E/M,then we can append 25 modifier. Some payers will pay this claim without adding 25 modifier,but some payers its mandatory to append 25 modifier even its not significant.
