I have a patient with a hx of Bladder CA. He came in for his yearly cystoscopy. In the HPI there is a summary of the CA and then "he has nocturia x 1. He void with a fair force stream. He denies dysuria, hematuria or any significant urgency". The cysto is performed and f/u in one year.

Does the HPI substantiate a 99212 due to the nocturia? The only dx coded was V10.51.

I don't think this warrants an E/M service. The patient did not come in for this symptom and it was not worked up.The patient came in for procedure.
