Is it appropriate for a DO to bill for an E/M (w/ modifier 25) every time they perform OMT (Osteopathic Manipulation Therapy) services?

The documentation may validate the E/M, but is it likely that the E/M is "medically necessary" every time?

if the patient is coming in specifically for the OMT, I would not bill an E/M...

UNLESS...there were other conditions assessed.

if the patient is coming in for pain and it is being fully assessed before the provider decides to perform the OMT, there may be a billable E/M.

I always look at it case by case to see what the documentation will support.
