Wiki E/M billing with same day procedures

Orthopedic practice here. We continue to get denials specifically from Aetna on E/M billing with same day procedures. Who else is experiencing this?

Aetna did send out a news letter back in 2020 stating new edits for modifier 25 - the denial means the required the progress note to support the used of the modifier 25
