Wiki E/M charge for follow up care?

As I understand according to MCR guidelines E/M charge can not be billed separately for a follow up care once the radiation oncology treatment has been completed. It says that follow up care is included into 90 days global period and will not be paid separately.

However, I see that follow E/M charges being billed and PAID all the time... Does anyone know specific guidelines?

I appreciate any help!
