Wiki E/M code with 11750 and 11732

I tired billing the following :
Line one - 99203 25 DX: B35.3
Line 2- 11750 59, TA DX: L60.0
Line 3- 11732 T1 Dx: L60.0
It came back as " Missing or invalid information.; P4999ADODN SmartEdit (ADODN) Procedure 11732 is an add-on code and must be reported with the primary code. It is recommended the Add-on and primary code be reported on the same claim form. Update code(s) as applicable for services rendered. "

Any helpful information would be great. Thanks

It's exactly what the SmartEdit states.

CPT 11732 is an add-on code. If you look in the CPT book, you'll see that there's a Code First note for 11730.

Report 11730 if only one nail plate is removed. Report 11732 for each additional nail plate removed.
