In the last couple weeks when 92250 is billing with an E/M code, the E/M code is getting denied due to being included in the global period. When the insurance company is contacted, they are saying that 92250 has a global period and the E/M code needs a 25. 92250 is a diagnostic test and does not have a global period. Our appeals are not being overturned. Did something change? I have not been able to find anything on this change occurring. Is anyone else having this issue?
