Wiki E/M codes vs eye exam codes

I have always worked in specialties other than opthalmology. So I am reaching out to the eye coders out there. In an opthalmology practice is it ever appropriate to use a 99xxx e/m code instead of the 92xxx codes?

A couple examples of scenarios would be helpful along with example of documentation guidelines.
 
Eye coding

Sure there are times...say someone comes in because they have something in their eye, has eye pain, has irritation, etc... The only time you use the 92XXX codes is when a medical eye exam is performed. If routine eye exam is performed then use the S codes. Hope this helps!
