Wiki E/M codes with H0020 in WA medicaid

Does anyone know if H0020 should be billed alongside a E/M code such as 99214 or if it should be billed on its own or alongside case management?

I am just lost in this. I am looking at the medicaid guidelines and I just can't tell.
