Wiki E/M codes with preventive codes unsure? HELP

I auditing e/m codes. The e/m codes are listed plus a modifier with a preventive code. Also pt has V 72.31 routine gyn exam. On some there is no documentation of what procedure was order but do have results. On others medications were given. I am confused. I have many of them. We are back logged and need to complete as many as we can with good accuracy and time. Help
