Wiki E/M coding quiestion

My teacher tried to explain to me how the E/M codes change when all of the key components don't match. You're supposed to bump something down? Hopefully someone knows what i'm talking about because I can't really explain it because I don't understand it.
 
If you need all three key components and they don't match you have to go with the one that is lowest as you have to meet or exceed all elements. if you only need two of the three elements, and none match you can throw out the lowest and use the middle one as your level of service. all of this is of course based on medical necessity of the documentation. I hope this helps
