Do you HAVE to have a physical exam for an est level 3 or 4? I know as long as u have 2 out of the 3 key components. Please help asap!! Thank you!!!

For an established patient only 2 out of 3 are required so an exam is not required for leveling. While it is questionable from an auditing stand point for coding you are ok to level from the hx and mdm.

For an est pt you only have to meet 2 out of 3. So, as long as your history and MDM meet requirements you can code it. Hope this helps!
