I have a phys who documents under Impression and Plans - " High Chol - Stable- continue with meds." Can I use this statement as a diagnosis or must phys state the actual diagnosis of "Hypercholesterolemia"? This provider always uses High Chol under the Impression & Plan in the documentation. Let me know your thoughts!:)
