Wiki E/M for twins under mothers id

How should E/M should be billed when the newborns are under the mother's id number and they're twins? Should two separate claims be billed? Should modifier 25 be used on one of the claims? Or should the services be billed on the same claim using a particular modifier?

I also code for peds - when twins are still under mother, we bill 1 claim for each baby no modifiers to mom's insurance. As babies are two separate individuals, insurance did pay.
