I have a patient that was in the hospital. He went home for a few days, then he was readmitted. I billed the readmission as an Initial Hospital visit - 99223, but my claim is being denied. Codes - 16 - Claim/service lacks information which is needed for adjudication". I can't figure out what I am missing. M81 - You are required to code to the highest level of specificity. My DX codes are R10.84, K50.114, L02.211.
Are there "time limits" on Initial hospital visit codes, just like on office codes - ie you can only bill 99204 once in 3 years. Any assistance would be appreciated. Thank you
