If the pt comes in with possible UTI symptoms, labs were done in house, and prescription prescribed.

In the dictation, does the provider still needs to dictate why he is ordering the test and dictate the results of the test such as positive/negative even though the actual test results is added in the dictation?

Please clarify? thanks
