Provider coding lower level to documentation points supports so I will like to know when Documentation supports a higher level and Medical Necessity the lower level what action best suggested. Code to level documentation points system supports OR Code to lower level as medical necessity OR discusses to provider to document per medical necessity.

If your History and Exam points support the higher level, and this is an established patient, you can use the higher level.
