What level would you assign? Documentation meets all requirements for 99284 - Work-up is single x-ray, 1 PO Norco and RX Ultram on D/C DX is hip pain

For MDM...
New Problem - Moderate

Risk - Moderate
So for 2 out of 3 your overall level for MDM would be Moderate which is eligible for either 99283 or 99284. For an overall level of 99284 it would depend on the presenting problem of whether it was Moderate or High. For moderate I would code 99283. For high I would code 99284.
