I've come across a few physical exams that the elements were not all there, but the provider clearly documented that the exam was unable to be completed for whatever reason. (Outpatient New Patient) Can I count the documentation as the provider attempted but unable to perform? For example: Musculoskeletal-unable to examine due to patient's arm in a cast?
