Good Afternoon,

One of my providers had a follow up appointment that took about 2 hours. I know we can bill 99215 for the first 40 minutes. Can we bill the remaining 80 minutes? Appreciate an help you all can give.
 
You can use prolonged time codes as long as the documentation supports this. First though you must assign the visit level that is met by the three key components and go from there. For example if the visit was mostly counseling you may say a 99213 met by the key components so you start there.
take the total time documented in minutes so in this case 120 and subtract the visit level met say 99213 which is worth 15 minutes
120-15 = 105
for the first 30 to 60 minutes beyond the visit you code a 99354
then you must be 15 minutes beyond the 60 to get the next level so at 75 minutes you may add on a 99355
that will take you 104 minutes, at 105 you bill another 99355 so it would look like this
99213
99354
99355 x 2
This is all payable by most every payer you just need most excellent dx codes to show medical necessity
