Question E/M spanning two doctors scheduled hours in UCC setting

Our UCC setting has two ARNP's. We are trying to determine the following "If a UCC provider(ARNP) initiates a visit with a patient documents the history and physical and creates a working diagnosis with a plan that is contingent on the likely result of a test to be done at the UCC that day, however the test is delayed and the provider of the latter half of that day’s shift(another ARNP) is left to review the results of the test, possibly change the diagnosis and plan then discharge the patient, does that patient need two separately documented notes" This doesn't seem to fall under shared services. I am not sure where to go for this?

Any suggestions appreciated. Thank you!
