Is there a generic template one can use for E&M and teaching MD's what must be documented to code consults?

Online E/M Interactive Worksheet

Hi Stephanie,

I found this First Coast (Medicare Carrier) E/M Interactive Worksheet online. You only have the options to select from three states (FL, PR or U.S. Virgin Islands), although I'm in CA, I just select FL, Part B since this worksheet contains a general multiple organ system tool.
