Can anyone further explain the guidance that was published for NCCI edits (January 2013) as it related to modifier 25 usage for physician billing.

Can a physician bill an E&M level with a casting procedure (e.g. serial clubfoot casting) if the physician performs an elaborate physical and then determines casting is necessary? The documentation has a gray area, it appears you can and also looks like you can't (page 4 of the spcified documentation). Please advise. Thanks.
