I know I need mod 25 for the E/M. Do I need one for 62270 as well?

Is this all that was documented? There's nothing there to support an E/M service. IF there was, the 25 would go on the E/M only, not the procedure code.
 
He did a full note for the E/M. That is what he documented about the procedure in the note. Also, there is a separate report for the procedure. I was just not sure because the procedure was unsuccessful, if I need any mod for 62270.
