curious if assessing chronic conditions is significant enough to warrant billing an E/M with an annual PE. Codapedia states “Patient reports no complaints” does not warrant an additional E/M service, even if the physician renews prescriptions and discusses chronic illnesses. Does the E/M have to address if patient has not responded to therapy, develops new symptoms, worsening symptoms, or new problems?

In order to code an office visit with the annual the provider must document abnormal findings on the examination. So reordering prescriptions and checking chronic conditions does not qualify as abnormal findings and therefore does not justify the addition of an office level.
