we have a provider who wants to code an E/M visit along with trigger Point injections. his documentation is limited with a brief history and limited review or symptoms, then it jumps to the procedure. i don't believe if the e/m and procedure were separated that the e/m would stand alone, but there are others in our system saying that yes he should get the e/m and procedure credit. i know that there is an e/m component in every procedure, and that its has to be above and beyond, but what justifies the above and beyond?
does saying that "patient doing well after the last injection, had little pain and was able to walk and perform all activities, then after Christmas she was taking down decorations and pain started to return,," justify an E/M?
