Pediatrics Office--- patient comes to get warts removed or have a umbilical cauterization could we also use the office vist code with modifier 25 for medical decision making the same day of procedure or should the office vist be something significantly different for the procedure
 
If they are only coming in for a wart removal then you code only the wart removal. If they come in for a separate visit like if they have a cold and the wart is there and they have it removed then you do the office visit with the mod -25. Or another example would be if the patient came in and wasn't sure if it was a wart or not and had it removed then you do the office visit with the mod 25 and the wart removal
