E/M -- it's ok if we bill under MDM but also add the time in the encounter (like the way reporting using time)?

Can you give an example? The E&M code can only be assigned based on MDM or time, so what do you mean by 'add the time'? Are you talking about billing the E&M service based on the MDM and then adding a prolonged services code?

If you are billing based on MDM the provider can include time in the note, however it is not recommended as it can cause confusion on what the reported code is based on. It is best to do one or the other, but not both so it is clear to an auditor or the insurance company etc. what the level of service being billed is based on if records are requested.
