Patient presents with Hyperglycemia. Patient has No previous history of Diabetes. Physician documents impression as New Onset Type 2 Diabetes Mellitus. What would be appropriate ICD code E11.9 or E11.65. Can you anyone help with ICD 10 CM guideline.?

E11.65 is appropriate if the provider has confirmed and documented that the patient was in fact hyperglycemic. If that's the case, then the combination code should be used, per the guidelines, unless the provider specifically states that the hyperglycemia was unrelated to diabetes.
