An internal medicine office I work with have always billed an E/M with CPT 69209 or 69210 if an ear lavage is performed. Recently, all 69209 are being denied. Are there any new guidelines that I am unaware of? Are others getting these denials? It's mostly Medicare or Humana.
Example: 99214-25
69209- 2 units is bilateral (this is what previous guidelines stated for Medicare)
