Can some one help me with getting echo paid when the having echos did for pre-chemo we was told to use 424.2, v58.11 and the cancer diagnosis in the past they were paying but now they are not. Can anybody give me advice for this one?
 
We use V82.1 and then the cancer diagnosis. We've only had trouble with a couple of insurance companies (United Healthcare and Oxford), but even they have paid on appeal.
